CONTRACT FOR LAB SERVICES

<br /> THIS AGREEMENT, sntered into as of this let day of July, 1991, by and between
<br /> Cabarrus Memorial Hospital (hereinafter referred to as CHH) and Cabarrus County
<br /> Department of ARint (hereinafter referred to as the "Purchaser").
<br />
<br /> WITNESSETH THAT:
<br />
<br /> ~S, the Purchaser desires to engage .C~ to render services in connection with
<br /> the Health $creenin~ Service for the older adults of Cabarrus County (hereinafter
<br /> referred to as "Pro, ram"), described in the Purchaser's contract with Centralina
<br /> council of Governments designated for the provision of Health Screening with Title
<br /> III funds to older adults provided to the Purchaser through Centralina Council of
<br /> Governments from North Carolina Division of Aging and the United States Department
<br /> of Health and Human Services; and,
<br />
<br /> W~S, CHH is a licensed service provider with the demonstrated capacity and
<br /> desire to render lab services for older adults;
<br />
<br /> NOW THEREFOP. E, in consideration of the premises and the mutual covenants and
<br /> agreements hereinafter contained, the parties hereto agree as follows:
<br />
<br /> 1. CHH agrees to sell and Purchaser agrees to buy lab services. C~ agrees to
<br /> provide service activities in accordance with standards North
<br /> as

<br /> Carolina Division of Aging. (Attached) C~ agrees not to subcontract for
<br /> the delivery of the services called for in this Agreement unless mutually
<br /> agreed upon by Purchaser and CMH.
<br />
<br />2. This Agreement will be in effect and services will be delivered at four (4)
<br /> fairs beginning September, 1991 and continuing through June 30, 1992.
<br />
<br />3. Purchaser will purchase up to 380 units within the stated contract period.
<br /> CMl{ shall be committed to develop the total units of services requested by
<br /> Purchaser at an average of 95 units screening.

<br />4A. CHH will deliver the lab package to each participant, screening each for:
<br /> Calcium, Phosphorus, Glucose, BUN, Uric Acid, Cholesterol, Protein, Albumin,
<br /> Bilirvbin, Alkaline Phosphatase, LDR, SGOT, T-4, White Blood Cotmt, Red Blood
<br /> Count, Hemoglobin, Hematocrit, MCV, MCH, MCHC, Polys, Lymphs, Monocyles, EOS,
<br /> BASO, Polys (Absolute Value) Pletelets, Urinalysis, Gross Exam, SPecific
<br /> Gravity, PH, Urine Color Appearance, WBC, Erterase, Protein, Glucose,
<br /> Acetone, Occult Blood, Microscopic Examination, if indicated with WBC/HBF,
<br /> RFC/H?F, Epilhelisl Cells Bacteria. Also Pap Smear screenings will be
<br /> secured by C~ st a later date for participants within two weeks of each
<br /> fair.
<br />
<br />4B. The number of pap smears shall not exceed 20Z of the total number of units.
<br /> Pep smears shall be exclusively for senior ladies who have never had a
<br /> hysterectomy, nor have had a pap smear within the last year.

<br /> shall be as follows: a unit price of $10.50 per participant with the
<br /> responsibility outlined in this contract.

<br />SB. If the number of pap smears exceeds 20I or 76 units per year, C~. shell hill
<br /> the Purchaser a pep unit price of $8.00 in addition to the unit price of
<br /> $10.50 for each unit served above the 76 units.
