POPUL~TION TO BE SF~RVED <br /> <br /> Any resident of Cabarrus County who is: <br /> A. 60 of and older or the <br /> years <br /> age <br /> spouse. <br /> <br /> .There shall be no discrimination with regard to race, sex, creed, or national <br /> origin. Priority is targeted toward: <br /> <br /> A. Older adults whom have not seen a physician in the past year. <br /> B. Older adults whom have visited a physician in the past year for an <br /> illness but did not receive a comprehensive physical exam. <br /> C, Older adults with limited economical and social supports. <br /> D. Older adults whom request the health screening service. <br /> <br />7. CM~ will agree to date and time of health fair with a minimum of 50 work days <br /> prior notice to tentative date. <br /> <br />8. CMH agrees to meet with Purchaser one time per quarter (or more frequently, <br /> if requested) to evaluate program operation and quality of service. Meetings <br /> shall he scheduled. <br /> <br />9. CMM shall furnish the Purchaser with a monthly program report and request for <br /> full payment of units of service delivered. Such reports shall be furnished <br /> within three (3) working days after the end of each month of screening. <br /> Purchaser shall remit payment in full within 30 days of receipt of request. <br /> <br />10. Interest of CMH: CMH covenants that neither CMH nor its agents or employees - <br /> presently has mn interest, nor shall acquire an interest, direct or indirect, <br /> which conflicts in any manner or degree with the performance of its service <br /> hereunder, or which would prevent, or tend to prevent, the satisfactory <br /> performance of the CMM service hereunder in an impartial and unbiased manner. <br /> CHH further covenants that in the performance of this contract no person <br /> having any such interest shall be employed by CMl{ as an agent, subcontractor <br /> or otherwise. <br /> <br />ll. Interest of Members of the ProRram and Others: No officer, member, or <br /> employee of the Program, and no public official of any local government which <br /> is affected in any way by the Program, who exercises any function or <br /> responsibilities in the review or approval of the Program or any component <br /> part thereof, shall participate in any decisions relating to this contract <br /> which affects his personal interest or the interest of any corporation, <br /> partnership or association in which he is, directly or indirectly interested; <br /> nor shall any such person have any interest, direct or indirect, in this <br /> contract or the proceeds arising therefrom. <br /> <br />12. Officials not to benefit: No member of or delesata to the Congress for the <br /> United States of America, resident Co~issioner or employee of the United <br /> States Government, shall be entitled to any share or part of this contract or <br /> any benefits to arise herefrom. <br /> <br /> <br />
