Box 35008 <br /> Charlotte, NC 28235 <br /> <br /> Dear Ms. Archer: <br /> <br /> Earlier this quarter we requested an extension of due dates for subcontracts. <br /> We have enclosed copies of the following respite care subcontracts: <br /> <br /> First Assembly Day Care for adult day care <br /> LIFE Center for adult day care <br /> Giles Leisure Home for temporary placement <br /> <br /> We have also enclosed the subcontract with Cabarrus Memorial Hospital for lab <br /> services for health screening, <br /> <br /> Unfortunately due to circumstances beyond our control we are not able to submit <br /> subcontract for CHORE services. We anticipate our County Commissioners to <br /> review these and approve and sign at their next meeting scheduled for January <br /> 2nd. We shall remit copies of these after signatures are obtained. We have <br /> enclosed a copy of the unsigned subcontract for your convenience. <br /> <br /> Also, we are requesting that you accept a subcontract for adult day health with <br /> First Assembly Adult Dsy Health for respite services. This is a new alternat~ <br /> for caregivers with patients who do not meet eligibility criteria for adult day <br /> care. This facility opened November 29, 1989. Enclosed is the potential <br /> contract for your review. Please note that the hourly cost will be prorated so <br /> as not to exceed the $26.00 maximum medicaid rate of reimbursement. <br /> <br /> If we can be of additional assistance please call us at (704) 788-9899.
