EAS, D.S.S. is a public agency with the demonstrated capacity and desire <br /> to render Chore services to older adults; <br /> <br /> NOW THEREFORE, In consideration of the premises and the mutual covenants and <br /> <br />1. D.S.S. agrees to sell and Purchaser agrees to buy Chore Service. D.S.S. <br /> <br /> by North Carolina Division of Aging. (Attached). D.S.S. agrees not' to sub- <br /> contract for the delivery of the services called for in this Agreement unless <br /> mutually agreed by Purchaser and D.S.S. <br /> <br /> 2.This'Agreement will be'in effect and services will be delivered beginning <br /> Ju~ 1, 1989, '.: and continuin~'th~oug~ June 30, 19~0. <br /> <br />3. Purchaser will purchase 6376 units pf s}rvic~wj~hin the <br /> stated contract period. D.S~S. shall be ~ommit~ed to <br /> deliver the total units of service request b~ the <br /> Purchaser within stated period. <br /> D.S.S. will deliver the Program based on one hour unit of service. Program <br /> will be 'provided in the home of the client. Service activities contingent <br /> upon participant ne~d as appropriate professional determines in plan of care. <br /> (Service Activiti~s attached). <br /> <br />5. D.S.S. mus£ retain copies of units of service. The units must be documented <br /> by time-of-day with documentation of allowable service activities. Cabarrus <br /> <br /> Records mu~t be signed by participant and Chore worker. <br /> <br />6. The purchase and delivery price for all units of service ordered by Pnrchaser <br /> shall be as follows: a unit price of $ 4.75 with the responsibility <br /> outlined in this contract. The purchase price shall include all expenses <br /> of the service delivery as defined in standards. <br /> <br /> <br />
