7. D.S.S. shall comply with the Older Americans Act in regard to target populations <br /> which emphasizes the socially needy and economically needy with special emphasis <br /> on low income minority elderly, The North Carolina Division of Aging is <br /> emphasizing rural elderly. These target groups shall be identified attempts <br /> to aerve the group documented and procedures shall be established and submitted <br /> to the Purchaser stating how the target group participants wll be given <br /> priority far receiving the service. <br /> 8.DSS shall comply with the following: <br /> DELIVERY CHARACTERISTICS: <br /> <br /> A. An'in-home assessment must be made by an appropriate D.S.S. staff member <br /> within 30 days of referral. <br /> <br /> B. Participant must be notified by D.S.S. in writing of their eligibility or <br /> ineligibility for Chore services within 20 working days of the in-home assessment. <br /> <br /> C. D.S.S. will develop and maintain a current written plan of care for each <br /> participant. <br /> <br /> D. Each participant must be reassessed by D.S.S. at least every six montha or <br /> more often aa indicated by the participant's condition by a social worker <br /> or their professional. <br /> <br />E. D.S.S. should be cognizant that clients should be assisted to become self- <br /> sufficient and capable of living safely and independently of Chore service <br /> whenever possible. This philosophy will enable service to be expanded to <br /> assist other "at risk" clients. <br /> <br />F. Chore providers will be supervised by D.S.S. <br /> <br />G. Chore provider and participant muat enter into a signed agreement witnessed <br /> byD.S.S. <br /> <br /> H. All Chore workers/providers must have at least three (3) hours of orientation <br /> training before going on the job. <br /> <br />I. Chore workers who perform Home Management and/or Basic Personal Care Tasks <br /> must complete at least thirty C30) hours of training (including tile orientation, <br /> with primary emphasis on personal care, home management and nutrition topics) <br /> within their first year of employment; workers providing Basic Personal Care <br /> Tasks (Level I) must complete the first fifteen hoars in their first three <br /> months of employment. If feasible, sixty (60) hours of training is recommended. <br /> <br />J. D.S.S. must assure that Chore worker is capable of performing (or has h~d' on- <br /> the-job training for) the specific tasks required for aa assigned client <br /> before that worker is allowed to work independently. <br /> <br />K. Before Chore workers are assigned Specialized Personal Care Tasks (Level <br /> II) they must have successfully completed at least nine (9) hours of personal <br /> care/nutrition training (may be part of the above basic 30 hours), plus any <br /> additional hour~ of specialized training necesmary to succe8sfully perform <br /> the assigned tasks.
