X. D.S.S. shall also meet these additonal requirements: <br /> <br /> I) Procedures for collecting Program Income, purchaser shall furnish envelopes. <br /> 2) Current job descriptions for each staff member involved in the service. <br /> 3) Procedures for evaluation of the service by participants (at least annually). <br /> Training plan for all staff involved with service. <br /> 5) Grievance procedures for all participants. <br /> 6) Policy for termination of service. <br /> 7) Policy fo? reporting changes in participants condition. <br /> 8) Written plan for how the agency will provide general and service, specific <br /> information and procedures for referral. <br /> 9) Written plan and documentation of outreach activities. <br /> <br /> Y. D.S.S. is required to provide the following reports to the Purchaser by the <br /> designated date of each month: <br /> <br /> 1) Completed MIS Client Registration Forms, <br /> 2) Completed requests for reimbursement. <br /> a) Requests for reimbursement shall consist of a summary sheet prepared by <br /> D.S.S. Back up documentation will be maintained at D.S;S. which consists <br /> of a signed statement by worker and participant and/or guardian, unpaid <br /> careglver which state hours and duties of service. Cabarrus County Time <br /> Records are not sufficient. <br /> <br /> 9o D.S.S. agrees to meet with Purchaser one time per quarter and including necessary <br /> follow-up assessment as in compliance with DOA and COG to evaluate Chore Service <br /> Program operation and quality of service. Meetings shall be scheduled. Ail <br /> units of service shall meet the required minimum service standards as adopted <br /> and implemented by North Carolina Division of Aging. <br /> <br />10. Ail referrals for the direct service provision shall be referred to D.S.S. <br /> Each case will be evaluated by D.S.S. and referred appropriately to Program. <br /> <br />Il. D.S.S. shall furnish tile purchaser with a monthly program report and request <br /> for paYment of units of service delivered. Such reports shall be furnished <br /> within 10 working days after the end of each month. A client ~.01 Registration <br /> Form must be submitted with reports provided as a courtesy by tile Purchaser. <br /> <br />12. D.S.S. shall encourage participants to make monthly contributions based on sliding <br /> fee scale. Contributions shall be mailed or delivered to Cabarrus County Depart- <br /> ment of Aging. No participant is denied services due to inability to coutribute. <br /> <br />13. D.S.S. shall comply an~ maintain documentation ascertaining selection criteria stated <br /> the North Carolina Division of Social Services Family Service Manual Volume V Chapter <br /> I which states: "since Chore services is (are) designed to support family care of <br /> individuals rather than supplant it, and because tile health and safety of the clients <br /> are involved, the following regulations must be applied in hiring chore~wor~ers: <br /> a. Persons who are selected must be: <br /> (1) Non-relatives who are aged 18 and over and who are qualified to perfo~ the <br /> tasks needed by the client; <br /> OR <br /> (2) Relatives of the client (parent, spouse, child or sibling) aged 18 and <br /> over who give up employment or tile opportunity of employment in order to perform <br /> the tasks needed by the client. <br /> b. Persons who may NOT be selected are: <br /> (1) Persons under age 18; <br /> (2)Persons who are not qualified to perform the tasks needed by the client: <br /> (3) Relatives of theclient (parent, spouse, child,or sibling) who are unemployed or <br /> who do not have to give up employment in order to provide tile service. <br /> <br /> <br />
