AGREEMENT FOR DRUG SCREENING SERVICES <br /> <br /> THIS AGREEMENT, made this ____ day o~ , 1990, by <br /> and between Wolfe & Associates, Inc., 26 Ardsley Avenue, NE, <br /> Concord, North Carolina, 2~025 and Cabarrus County {hereinafter <br /> "CLIENT"). <br /> <br /> This agreement shall be enacted for the one year period <br /> commencing April 1, 1990 and will automatically renew for <br /> additional one year periods at the end of its term unless either <br /> party has given written notice of intent to change the terms of <br /> the agreement no less than sixty (60) days prior to the renewal <br /> date. <br /> <br /> ARTICLE 1: PRICES <br /> <br /> In consideration of payment for services rendered according to <br /> the attached schedule of fees (Appendix A - hereby incorporated <br /> by reference into this agreement), CLIENT employs Wolfe & <br /> Associates to provide drug screening services as defined in <br /> Article 2 below. Prices quoted in Appeadix A will hold <br /> throughout the term of this contract; such prices are subject to <br /> revision only in the event of the enactment of additional <br /> government regulations which significantly alter the requirements <br /> for provision of drug screening services. If such regulatory <br /> changes occur, prices are subject to change by Wolfe & Associates <br /> for sixty days from the effective date of such changes. CLIENT <br /> has the option .of refusing to continue to receive services from <br /> wolfe & Associates under new pricing agreements. <br /> <br /> ARTICLE 2: SERVICES RENQERED <br /> <br /> Services to be performed by Wolfe & Associates for CLIENT <br /> include: <br /> <br />1. Contract wit|] CompuChem Laboratory for drug screening <br /> services. Review for the presence or absence of five drugs <br /> will be execated: amphetaminest cocaine, marijuana, opiates, <br /> and phencyclidine (PCP). If CompuChem loses NIDA <br /> certification or if the MRO determines the quality of testing <br /> is not satisfactory then an alternate NIDA approved lab will <br /> be contracted with by Wolfe & Associates for the CLIENT. The <br /> selected lab must be a~proved by the CLIENT. <br /> <br />2. Supply CLIENT collection locations with chain-of-custody <br /> forms, collection kits, airbills, and necessary instructions <br /> for proper collection of urine samples. <br /> <br /> (1) <br /> <br /> <br />
