4a: CLIENT acknowledges that performance of necessary verification procedures is dependent upo~ cooperation by CLIENT representatives, employees or applicants, and/or the personal physicians and/or health care providers that possess vital medical history information. 4b: CLIENT agrees to provide a designated representative that will receive all reports of test results, assist the MRO in the collection of information necessary for result review, and be responsible to arrange for an interview between the MRO and employee or applicant. 4c: CLIENT agrees to provide security measures and appropriate training of all personnel involved in drug testing procedures to assure that strict confidentiality of results is maintained to protect the dignity and personal rights to privacy of tested individuals. 5. The MRO will provide a toll-free WATTS line which will be available for use by CLIENT, CLIENT's employees or applicants for employment, or health care providers providing information on CLIENT's employees or applicants for employment. 6. The MRO will provide to CLIENT testing and quality control information. Positive results will be stored for at least five years, negative results for at least one year by CLIENT. 7. Wolfe & Associates will provide drug screening services for CLIENT which will be in compliance with the Administrative Rules and Implementation Guide as adopted by the North Carolina Sheriffs' Education and Training Standards Commission. ARTICLE 3: ADDITIONAL SERVICES Additional services to be provided by Wolfe & Associates to CLIENT at rates according to Appendix B (hereby incorporated by reference into this agreement) include: 1. Documentation of results and procedures for judicial and quasi-judicial proceedings. This provides CLIENT with all results, chain-of-custody documents, operating procedures, and relevant biographical information on personnel participating in specimen review. This is available only upon written request.
