4a: CLIENT acknowledges that performance of necessary <br /> verification procedures is dependent upo~ cooperation by <br /> CLIENT representatives, employees or applicants, and/or <br /> the personal physicians and/or health care providers <br /> that possess vital medical history information. <br /> <br /> 4b: CLIENT agrees to provide a designated representative <br /> that will receive all reports of test results, <br /> assist the MRO in the collection of information <br /> necessary for result review, and be responsible <br /> to arrange for an interview between the MRO and employee <br /> or applicant. <br /> <br /> 4c: CLIENT agrees to provide security measures and <br /> appropriate training of all personnel involved in <br /> drug testing procedures to assure that strict <br /> confidentiality of results is maintained to protect <br /> the dignity and personal rights to privacy of tested <br /> individuals. <br /> <br /> 5. The MRO will provide a toll-free WATTS line <br /> which will be available for use by CLIENT, CLIENT's <br /> employees or applicants for employment, or health care <br /> providers providing information on CLIENT's employees or <br /> applicants for employment. <br /> <br />6. The MRO will provide to CLIENT testing and quality control <br /> information. Positive results will be stored for at least <br /> five years, negative results for at least one year by CLIENT. <br /> <br />7. Wolfe & Associates will provide drug screening services for <br /> CLIENT which will be in compliance with the Administrative <br /> Rules and Implementation Guide as adopted by the North <br /> Carolina Sheriffs' Education and Training Standards <br /> Commission. <br /> <br />ARTICLE 3: ADDITIONAL SERVICES <br /> <br />Additional services to be provided by Wolfe & Associates to <br />CLIENT at rates according to Appendix B (hereby incorporated by <br />reference into this agreement) include: <br /> <br />1. Documentation of results and procedures for judicial and <br /> quasi-judicial proceedings. This provides CLIENT with <br /> all results, chain-of-custody documents, operating <br /> procedures, and relevant biographical information on <br /> personnel participating in specimen review. This is <br /> available only upon written request. <br /> <br /> <br />
