Page 3, I <br /> <br />MEASURABLE OBJECTIVE(S): <br /> <br /> 1. Provide an emergency residence for 85% of all childrua determined <br /> appropriate for placement. <br /> <br /> 2. To provide a licensed group home for 5 childres supervised by Lutheran <br /> Family Services staff. <br /> <br /> All youths will be in DSS custody. <br /> 4. Ail youths will have medical examinations within 10 days of placement. <br /> <br />5. Ail youths will have psychological evaluation unless recent (within tile <br /> last six months) prior to placement. <br /> <br />6. All youths ~xperiencing substance abuse will be referred to substance <br /> abuse screening and follow up counseling when indicated. <br /> <br />7. Ali youths will be enrolled in a school system and educational assess- <br /> ments will be done with the school to determine tile most appropriate <br /> educational setting for the child. <br /> <br />8. 60% of all youths served by the program will be prepared for long term <br /> placement within 90 days of the entry date (preparation is defined as <br /> st~bilization of the child's behavior to a point where he is ready for <br /> placement and development of a long term care plan). <br /> <br />9. Of the children completing the 90 day program, 50% will achieve <br /> successful adjustment for 6 months following long term placement. <br /> (Success shall be defined as: Ill The child does not come to the <br /> attention of the ~uveuile Court; [2] The child, the social worker, and <br /> his legal guardian agree that placement was successful 6 months after <br /> it was mede.) <br /> <br />INFORMATION MAINTAINED FOR EFFECTIVENESS MEASUREMENT: <br /> <br /> Child's file will be maintained at the group home containing admission <br /> form, student intake record, parental consent for nonprescription <br /> medication, social summary, student visitation forms, plan of care <br /> forms, staff'log sheets regarding each child. <br /> <br /> Department of Social Services will keep a folder on each child <br /> containing court order showing custody with Department of Social <br /> Services, termination summary on the child, case dictatios involving <br /> family counseling and coordination of services to the child and family, <br /> 6 month follow up report after discharge from thc home, weekly progress <br /> reports submitted by the home to DSS, client tracking forms submitted <br /> to CBA office. <br /> <br />3. Budget information documenting in-kind and cas|* flow kept by both the <br /> home and DSS. <br /> <br />4. Staff work schedule kept by the home. <br /> <br /> <br />
