~iENDATORY RIDER TO MASTER GROUP CONTRACT between BLUE CROSS AND BLUE SHIELD OF NORTH CAROLINA, AND CABARRUS COUNTY EHPLOYEES of Concord , North Carolina In consideration of pa)q~ent of appropriate subscription fees, the contract beL~'een the above two parties is amended as follows: I. A. Beginnin§ with th~ effective date of this Amendatory Rider, ratfn§ under the Contract will be subject to the terms of the attached Hodified Minimum PremiumRating Agreement, Section H126A-~, which is hereby made a part of the Contract by reference as fully as if set forth therein. B. Accounting rill not be continuous between the Modified Retrospective Rating Agreement and the Modified Minimum Premium Rating A§reement. II. Section H126A-DE, DEFINITIONS, of the Contract is amended on page H126A-DE-5 by deleting the paragraph entitled "Pre-Existing Condition" in its entirety and substituting the following paragraph entitled "Pre-Existing Condition". Pre-~xistinK Conditio~ - those conditions for t~hich medical advice or treatment was received or recommended or ~hich could be immediately documented with the 12 month period immediately precedin§ the effective date of the Member's coverage. III. The Contract is a~ended by deletin§ Sections H126B-CD, Schedule of Benefits for Chemical Dependency Treatment Benefits and H126A-CD, Chemical Dependency Treatment Benefits, and substitutin~ the attached Sections M126B-CD, Schedule of Benefits for Chemical Dependency Treatment Benefits and H126A-CD, Chemical Dependency Treatment B~nefits. Other than as changed and amended, the remaining terms and conditions of the Haster Group Contract shall remain in full force and effect unchanged.
