CllEHICAL DEPENDENCY TRI~TMENT BENEFITS <br /> <br /> Subject to the Exclusions, General Provisions, conditions and limitations of <br /> this Contract, a Member is entitled to the benefits of this benefit section as <br /> specified in the Schedule of Benefits. <br /> <br /> Treat~=ent and rehabilitation care provided to a Member with a prlma~y dis~nosis <br /> of chemical dependency or abuse by a licensed or certified institutional or <br /> professional provider. <br /> <br /> A. INPATIENT/RESIDENTIAL ~EHABILITATION SERV~CES <br /> <br /> 1. Bed, Board and General Nursing Service in a semiprivate accommodation. <br /> However, if a Hember is admitted to a facility that has only private <br /> rooms, the most prevalent private room rate, as deter=lned by the <br /> Plan, will be considered semiprivate for the purpose of providing <br /> benefits under the Contract. Room allowance includes Croup <br /> Psychotherpay and Occupational Therapy in addition to Bed, Board and <br /> General Nursing Care. <br /> <br /> 2. Ancillary Services <br /> <br /> a. Diagnostic Services <br /> <br /> b. Drugs, biologicals, and solutions dispensed by the facility <br /> and used while the Member is in the facility. <br /> <br /> c. Supplies and equipment required for rehabilitation. <br /> <br />B. PROFESSIONAL SERVICES <br /> <br /> 1. Services provided by a Doctor and certified professionals working <br /> under the direct supervision of a Doctor as follows: <br /> <br /> a. Psychological Testing <br /> <br /> b. Individual and Croup Psychotherapy <br /> <br /> c. Family Counseling designed to help treat the Member°s illness. <br /> <br />M126A-CD-1 <br /> <br /> <br />
