CABARRDS COUNTY AMENDMENT TO CONTRACT <br /> <br /> This Amendment, entered into this 13th day of September, 1990, by <br />Cabarr~s County Department of Aging (hereinafter called the "Purchaser") and <br />First Assembly Adult Day Care Center for adult day health services (hereinafter <br />called the "Center"). <br /> <br /> · . WITNESSETH THAT: <br /> <br /> ~rHEKEAS, the Purchaser and the Center into that certain agreement dated <br />July 1, 1990 pursuant to which the Canter was to provide day health services <br />for those parsons who qualify for respite care ss defined in the contrsct, and <br /> <br /> ~tEKEAS, certain changes need to he made in purchase and delivery price <br />for all units of service ordered by the Purchaser; <br /> <br /> NOW, THEREFORE, it is agreed that the aforesaid contract be and it is <br />hereby amended in the following respects: <br /> <br /> Item number five (5) is amended by deleting the unit price of $4.33 par <br />hour based on a six (6) hour respite stay and also deleting the pro-rate (in <br />the event that more or less than six (6) hours are contracted by the <br />participant and the Purchaser, the number of hours requested shall be divided <br />by 426.00 for the hourly unit rate thereby not exceeding the $26.00 per day <br />maximum) and'inserting in lieu thereof the following unit price of $26.00 <br />per day (with a six (6) hour minimum stay) shall be the purchase and delivery <br />price for all units of service ordered by the Porchaser. Monthly request for <br />payment by the Center shall include but not be limited to an individual <br />summary consisting of the dates and total hours not to exceed 42 hours per <br />month and the total amoun~ owed by the Purchaser. <br /> <br /> <br />
