CONTRACT FOR RESPIT£ CARE ADULT DAY CAR~ PACILIT~ l'~IS AGREEHEN~s entered into as of this 1st day of Jul~ ,1990 by and between L.I.F.E. Center (hereinafter referred as Center) and Cabarrus County Department of Aging, (hereinafter referred to aa the "Purchaser"). WITNE$SETH THAT: WHEREAS, the Purchaser desire to engage the L.I.P.E. Center to render set, ices in connection ~ith the Respite Care Program ~or the older adults of Caharrus County (hereinafter re~erred to as t~e "Prosram'), described in the Purchaser's contract ~ith CenCraltna Council of Cover~=ents designated leg the provision of respite care seruices through Title funds, $S~C funds and Se~ate Bill 15~9 funds co older adults provided to ~he ~u~chaser through Centralina Council of Gove~ments from ~orth Carolina Divisio~ of A§ing and U~ited States ~epart=enc of Realtb and ~umnn Se~-uices; and, ~ER]~$, ~.LP.~. Center is a llcensed adult da~ care semite provider ~£th the demonstrated capacit~ and desire co render adult d~ care secvlces to older adults; ~0~ TH£~O~£~ in constderatio~ n~ the premises a~d the mutual covenants ~d a§ree=ents hereln~fter contained, c~e ~arties hereto agree as 1. The Center agrees to sel! and Purchaser agrees to bu~ Center's respite services. ~he Center asrees to provide services and activities in accordance ~ith :standards as provided b~ R.C. Divison of (Attached) ~he Center agrees not to subcontract for the delivery the seruices called for in thls A~reement u~less mutually agreed Purchaser and the Center. 2. ~his Agreement ~ill be in effect and services ~ill be ~e~iveted beginning July 1~ 1990 an~ continuing through June 30. 1991 3. Purchaser ~tll purchase up to 576 unit hours vith no ~:e than 68 hours per client per ~onch ~ichin the stated contract period. The Center shall be co~itted co deliver the coral units of semites requested by the Purchase:. 6. The Center ~ill deliver the Program based on one (1) hour unit of semite. Program viii be provided in the adult day ca~e facility. 5. The purchaser and deli~e~ price for all units of semite ordered by Purchaser shall be as follo~s: a unit price of $ 3.25 per hour ~tch the responsibility outlined tn chis contract.
