APPENDIX H-1 <br /> <br /> CONTRACT FOR RESPITE CARE <br /> ADULT DAY CARE FACILITY <br /> <br /> Tills AGREEmeNT, entered into as of this 1st day of j~l? , <br /> by and be£ween First Assembly Adult Day Care (hereinafter referred <br /> to as the Center) and Cabarets County Departmant of Aging, (hereinafter <br /> referred to as the "Pure.baser"). <br /> <br /> ~IT~£SSETH T~AT: <br /> <br /> ~ERF-6g, the Purchaser desire to engage the First Assembly Adult Day <br /> Care Center to render services in connection ~ith the Respite Care Program <br /> for the older a~ults of Cabarrus Caunty (hereinafter referred ts ss the <br /> "Program"), described in the Purchaser's Contract with Centrallna Council <br /> of Governments designated for the provision of respite care services <br /> through Title III funds, $$BG funds and Senate Bill 1559 funds to older adults <br /> provided tn the Fa£chaser through Cantrelina Council of Governments from <br /> North Carolina Division of Aging and United States Department of Health <br /> and Human Secvices; and, <br /> <br />L~EREAS, First Assembly Adult Day C~re Center is a licensed adult day care <br />service provider with the demonstrated capacity and desire to render ~dult <br />day care services to older adults; <br /> <br />NO~ THEREFORE, in consideration of the premises and the ~ucual covenants <br />and agreements hereinafter contained, the parties hereto agree as <br /> <br /> l. The C~nrer agrees to sell and Purchaser agrees to buy Cenrer*s respite <br /> <br /> accordance with standards as provided by N.C. Division of Aging. <br /> (Actached) The Center agrees not to subcontract for the delivery of <br /> the services called for in this Agreemene unless ~utually agreed by <br /> Purchaser ~ rhe Center. <br /> <br />2. This Agreement will be in effect and services rill be delivered <br /> beginning July 1. 1990 and continuing through .1.ne ~. 1~ql <br /> <br />3. Purchaser will purchase up to 576 unit hours vith no more than ~8 hours <br /> per client per month ~ithin the stated contract period. The Center <br /> shall be ce~itted to deliver the total units of services requested by <br /> the Purchaser. <br /> <br />6. The Center will deliver the Program based on a one hour unit of service. <br /> Program vii1 be provided in the adult day care facili~y. <br /> <br />5. The purchase and delivery price for all units of ser~ice ordered by <br /> Purchaser sha31 be as follo~s: a unit price of $3.25 pet hour with the <br /> ~esponsihility outlined in this contract.
