Vincent <br /> <br /> and Cabarrus Coun~ Hea~Lh Department , here~na~Ce~ referred to as ~he <br /> A~ency. <br /> <br /> WITNESSETH: <br /> <br /> Whereas, the Agency has identified a need for medical consultation at <br /> the Caba~rus County Health Department, <br /> <br /> Whereas, it is to the Agency's interest, the community's benefit, and <br />the enhancement of patient care to provide medical consultation services as <br />herein specified; and, <br /> <br /> Whereas, Dr. J. Vincent Arey <br /> <br />is prepared to assume the responsibility for providing medical consultation <br />services; now therefore, <br /> <br /> IT IS AGREED BETWEEN THE AGENCY AND <br /> <br /> Dr. J. Vincent Arey as follows: <br /> <br /> 1. The ' physician <br /> <br />will provide patient care services to patienta at the Cabarrus County Health <br />Department. <br /> <br /> <br />
