2. These services will be provided on a contractual basis as follows: <br /> <br /> Mondays - Maternity Clinic: 1:00 PM - 4:00 PM (Concord) (12 hours/month) <br /> <br /> Tuesdays - Family Planning Clinic: 1st & 3rd Tuesdays (Kannapolis) <br /> 9:00 AM - 4:00 PM (12 hours/month) <br /> <br /> -Maternity Clinic: 2nd Tuesdays (Kannapolis) <br /> 9:00 AM - 4:00 PM (6 hours/month) <br /> <br /> Wednesdays - Family Plannning Clinic: 9:00 AM - 12:00 Noon/Concord <br /> (12 hours/month) <br /> <br /> Thursdays - Maternity Clinic: 9:00 All - 4:00 PM (Concord) <br /> On 3rd Thursday 9:00 AM - 12:00 Noon <br /> (21 hours/month) <br /> <br /> 3. In addition, the physician shall be responsible for signing any <br />patient records for patients seen at the health department by a physician <br />extender working under the supervision of the physician. <br /> <br /> 4. The Agency shall pay for medical consultation services pursuant to <br />this agreement rendered during the preceding month at a rate of $5,466.66 <br />per month to be paid by the 15th day of the month. <br /> <br /> 5, The duties and responsibilities of the physician staffing the health <br />department are those defined by the operating policies of tbe Agency. The <br />physician will maintain a current license to practice medicine in tba State of <br />North Carolina. <br /> <br /> 6. The physician shall maintain records and reports in accordance with <br />the policies of the health department. <br /> <br /> 7. The physician shall provide a current and valid certificate of <br />insurance evidencing that she/he has adequate professional liability insurance <br />coverage for services rendered to the Agency. This insurance coverage shall <br />be at the expense of the Agency and the physician shall reimburse the Agency <br />for any reimbursements the insurer shall make to the physician due to any <br />decrease in tho length of coverage. <br /> <br /> <br />
