Page 3 of 7 <br /> <br />:!~ g. ADMISSION AND DISCHARGE PROCESS <br /> a, The Center will confirm date, time of admission and discharge from <br /> <br /> b. Admissions must be lerner less than twenty-four hours nor f'or longer <br /> than 576 hours per client per contract period. <br /> <br /> 9. TheCenter agrees to meet with PUrchaser one time per quarter'(or more <br /> frequently, if requested) to evaluate respite service program operation <br /> and quality of service. Meetings shall be scheduled. All units of <br /> service ahall meet the required minimum service standards as adopted <br /> and implemented by North Carolina Division of Aging. <br /> <br /> 10. The Center shall furnish the Purchaser with a monthly program report <br /> and a request for payment of units of service delivered. Such reports <br /> shall be furnished within three (3) working days after tha end of each <br /> month. Request for payment must be itemized into units of service. <br /> <br /> 11. Interest of the Center: The Center covenants that neither the Center <br /> nor its agents or employees presently has au interest, nor shall acquire <br /> an interest, direct or indirect, which conflicts in any manner or dergree <br /> With the performance of its service hereunder, or Which would prevent~ <br /> or tend to prevent, the satisfactory performance of the Centerts service <br /> hereunder in an impartial and unbiased manner. The Center further <br /> covenants that in the performance of this contract no person having <br /> any seeh interest shall be employed by ~e Center as an agent, subcontractor <br /> or otherwise. <br /> <br /> 12, Interest of Members of the Pro,ram and Others. No officer, member, <br /> or employee of the Program, and no public official of any local government <br /> which is affected in'any way by the Program, who exercises any function <br /> Or responsibilities in the review or approval of the Program or <br /> component part thereof, shall participate in any decisions relating <br /> to this contract which affects his personal interest or the interest <br /> of any corporation, partnership or association in which he is, directly <br /> or indirectly interested; nor shall any such person have any interest, <br /> direct or indirect in this contract or the proceeds arising therefrom. <br /> <br /> 13. Officials Not to generiC: No. member of or 'delegate to the Congress <br /> for the United States of America, resident commissioner or employee <br /> of the United States Government, shall be entitled to any share or <br /> part of this contract or any benefits to ~rise herefrom. <br /> <br /> <br />
