C~barrus~Department of A~i_~, (hereinafter referred to as the "Purchaser )~ WiT~ESSET~THA~: W~, the purchaser desires to engage the L.I.F.E. Center to render services in connenetion with the Respite Care Program for the older adults of Cabarrus County (Hereinafter referred to as "Program"3, described in the PurchaSer's contract with Centralina Council of Governments designated for the provision of respite care services through Title III fund, SSBG fund and Senate Bill 1559 funds to older adults provided to the Purchaser through Centralina Council of Governments from North Carolina Division of Aging and United States Department o~ Health and Human Services; and, W~EREAS, L.I.F.E. Center is a licensed adult day care service provider with the demonstrated capacity and desire to render adult day care services to older adults; NOW T~FORE, in consideration of the premises and the mutual covenants and agreements hereinafter, contained, the parties hereto agree as follows: 1. The Center agrees to sell and Purchaser agrees to buy Center'S respite services. The Center agrees to provide services activities in accordance with standards as provided by N.C. Division of Aging. (Attached) The Center agrees not to sub-contract for the delivery of the services' called for in this Agreement unless mutually agreed by Purchaser and the Center. 2. This Agreement will be in effect and services will he delivered beginning October, 1989_ and continuing through June30,.l_~_~ 3. purchaser will purchase up to 576 ~nit hours with no more than 48 hours per client per month within the stated contract period. The Center shall be committed to deliver the total units of services requested by the Purchaser. 4. The Center will deliver the program based on one hour unit of service- Program will be provided in the adult day care facility. 5. The purchase and delivery price for all units of service ordered by purchaser shall be as follows: a unit price of $3.25) per hour with the responsibility outlined inthis contract.
