Administrative Requirements: Record maintenance of respite cases to <br /> include documentation of number of hours provided and evidence <br /> the Program was rendered and an evaluation of the service by the client. <br /> <br /> b.Any and all expenses incurred hy the Center for the provision <br /> of the Program. <br /> <br /> 6. POPLVuATION TO BE SERVED <br /> <br /> A~y resident of Cabarrus County who is: <br /> <br /> 60 years of age and older and/or their spouse. <br /> b. Physically and/or mentally impaired individual who cannot be left <br /> alone who require constant 24-hour supervision. <br /> c. Who are not elislble for Medicaid (unless individual is not eligible <br /> for respite care through the N. C. Medicaid Program in which event, <br /> units of service may not exceed 720. <br /> <br /> There shall be no discrimination with regard to race, sex, creed, or <br /> national origin. Admission for respite care will be based on caregiver's <br /> need and the appropriateness of the services offered by the Center to meet <br /> these needs. Priority is on first-come, first-serve basis. <br /> <br /> 7, PROCEDURES FOR REFERRAL FROM PURCI~SER <br /> <br /> a. Ail referrals for the direct service provision shall be referred to the <br /> Department of Aging. Each case will be evaluated by the Purchaser and <br /> referred to the center for the purchase of the service. <br /> <br /> b. Caregiver's requesting respite care will have an initial conference <br /> with the Center at least seven days prior to admission. <br /> <br /> c. The Center will be responsible for explanation of policies and <br /> procedures to caregivers during conference. <br /> <br /> 8. The Center agrees to meet with Purchaser one time per quarter (or <br /> more frequently, if requested) to evaluate Respite Service Program <br /> operation and quality of service. Meetings shall be scheduled at <br /> convenience of Purchaser and Center. Ail units of service shall meet <br /> the required minimum service standards as adopted and implemented <br /> by North Carolina Division of Aging. (Attached)° <br /> <br /> 9~ The Center shall furnish the Purchaser with a monthly program report <br /> and a request for payment of units of service delivered. Such reports <br /> shall be furnished within three (3) working days after the end of <br /> each month. <br /> <br /> 10. Interest of the Center: The Center covenants that neither the Center- <br /> nor its agents or employees presently has an interest, nor shall acquire <br /> an interest, direct or indirect, which cqnflicts in any manner or <br /> degree with the performance of its service hereunder, or which would <br /> .o preven~,~or tend to prevent, the satisfactory performance of the Centerts <br /> service hereunder in an impartial and unbiased manner.
