7. D.S.S. shall comply with the Older Americans Act in regard to target populations which emphasizes the socially needy and economically needy with special emphasis on low income'minority elderly. The North Carblina Division of Aging is emphasizing rural elderly. These target groups shall be identifiedattempts ts serve the group documented and procedures shall be established and submitted to the Purohaser stating how the target group participants wll be given priority for receiving the service. DELIVERY C}~RACTERISTICS: A. An in-home assessment must be made by an appropriate D.S.S. staff member within 30 days of referral. B. Participant must be notified by D.S.8. in writing of their eligibility or ineligibility for Chore services within 20 working days of the in-home assessment. C. D.S.$. will develop and maintain ~ current written plan of care for each participant. D. Each participant must be reassessed by D.S.S. at least every six months or more often as indicated by the participant's condition by a social worker or their professional. E. D.S.E. should be cognizant that clients should he assisted to become self- sufficient and capable of living safely and independently of Chore service whenever possible. This philosophy will enable service to be expanded to assist other "at risk" clients. F. Chore providers will be supervised by D.S.S. G. Chore provider and participant must enter into a signed agreement witnessed by H. Ail Chore workers/providers must have at least three (3) hours of orientation training before going on the job. I. Chore workers who perform Home Management and/or Basic Personal Care Tasks must complete at least thirty (30) hours of training (including the orientation, with primary emphasis on personal care, home management and nutrition topics) within their first year of employment; workers providing Basic Personal Care Tasks (Level I) must complete the~first fifteen hours in their first three months of employment. If feasible, sixty (60) hours of training is recommended. J. D.S.S. must assure that Chore worker is capable of performing (or has had on- the-job training for) the specific tasks required for as assigned client before that worker is allowed to,work independently. K. Before Chore workers are assigned Specialized Personal CareTasks (Level II) they must have successfully completed at least nine (9) hours of personal care/nutrition training (may be part of the above basic 30 hours), p~us any additional hours of specialized training necessary to successfully perform the assigned tasks. An additional nine (9) hours of personal cars/nutrition
