Monitoring visits must be made by D.S.S.professional staff each 3 month period <br /> in which an assessment visit £s not made. <br /> <br />STAFFING REQUIREMENTS: <br /> <br /> A Chore provider worker ~ust be 18 years of age or older. <br /> <br />ADmiNISTRATIVE ACTIVITIES: <br /> <br />M. D.S.S. shall have documentation of the following: <br /> <br /> (1) The original request for service received. <br /> (2) In-home assessment which provides data to clearly determine eligibility <br /> for the service and appropriate needs. <br /> (3) Written policy of eligibility or non-eligibility for Chore service. <br /> (4) Written notification to the client of eligibility or non-eligibility <br /> for the service. <br /> (5) A written Plan of Care. <br /> (6) A signed agreement. <br /> (7) Documentation of required training and orientation. <br /> (8) Documented reassessment every six months and a documented monitoring visit <br /> by the supervis~: each quarter a reassessment is not compl~te'd. <br /> <br />N. D.S.S. shall comply with all general administrative standards cited in the North <br /> Carolina'Division of Aging Policy and Procedure Manual as specified in contract. <br /> <br />O. D.S.S. shall'maintain records on all chore participants including dates of services, <br /> tasks completed, hours of service. <br /> <br />P. D.S.S. shall establish written agency procedures for reporti~lg changes in participant <br /> status as specified in Volume5, Chapte~}~.S.S. Manual. <br />Q. D.S.S. shall provide each chore provider (worker) with necessary training. <br /> <br />R.D.S.S. ~hall provide each chore provider (worker) with supervision. <br /> <br /> S. D.S.S. shall complete time sheets consistant with monthly report. <br /> <br />T. D.S.S. shall record maintenance of chore cases to include documentation of number <br /> of hours provided and evidence the Program was rendered and evaluation o~ the <br /> service by the participant. <br /> <br />U. Chore Provider: Travel and/or mileage reimbursement expense incurred by the <br /> provision of service delivery provided by D.S.S. <br /> <br /> V. Recruitment, hire, placement, training of all workers is responsibility of D.S.S. <br /> <br />W. Any and all expenses incurred by D.S.S. for the provisions of the program shall <br /> be the responsibility of D.S.S. <br /> <br /> <br />
