Cook, Jr. <br /> Director <br /> <br /> February 18, 1987 <br /> <br /> HEHORANDUH <br /> <br /> TO: Charles HcGinnis, County Hanager <br /> }'ROH: Jfln Cook, Social Service Director <br /> SUBJECT: Approval for Grant Application <br /> <br /> I would like to request that the Board of County Co~issioners at its Hatch <br /> 16 meeting allow DSS to apply for a Long Term Care Screening grant and designate <br /> the Department of Social Services as the lead agency to explore the development <br /> and implementation of the program in Cabarrus County. <br /> <br /> The LonE Term Care Screening Program (LTC) and Counnunity Alternatives <br />Program for Disabled Adults (CAP-DA) are part of legislation effective <br />in October 1981 thro:lgh House Bill 605 of the H. C. General Assembly. The <br />purpose of the program is to prevent institutionalization, where possible, <br />through use o£ a comprehensive screening program for older persons and <br />development of community resources to help disabled adults to remain in their own <br />homes. The screeninS of persons in need of Long Term Care is performed by a <br />nurse and a social worker team and consists of a home visit and consultation <br />with the client's physician. The focus of the team assessment is to determine if <br />in-home care may enable the person to stay at home or if n. ursing home placement <br />is most appropriate. The target population inaludes those who qualify for <br />admission to a skilled or intermediate level nursing facility and are Hedicaid <br />eligible or persons who may pay privately for screening. <br /> <br /> Since the legislation vas enacted 33 counties in N. C. have developed <br />screening programs. The Hedicaid waiver provisions of the law will allow <br />Hedicaid to cover screening cost as well es expenses associated with medical <br />supplies, chore services, nursing visits, and casework services. Host counties <br />have designated the local DSS as lead agency due to the social work staff use of <br />in-home services, close working relationships with the Hedtcaid unit, and <br />knowledge of Hedicaid eligibility requirements. <br /> <br /> ~e are seeking a State grant of $25,000 for each of the next two years in <br />order to start the program. No match is required for these funds. During the <br />two year period various services and Nedicaid reimbursement procedures will be <br />established in order for the program to be self-supporting. A Social Yorker II <br />will be needed to perform community education duties, determine client <br />eligibility, provide assessments, counsel with clients aug their families, <br />supervise Chore work staff, and coordinate overall efforts w/th other cou~munity <br />agencies. A Clerk lypist 111 will also be needed to manage the ltedicaid billing <br />and reimbursement procedures. It is estimated that a total of 30 individual <br />adults can be served in the first year of the program. Since chore services viii <br />be a major Coamunity Alternative Program service for these clients, ve would like <br />to consider the use of a flat rate exempt method of payment for chore workers. <br /> <br /> <br />
