PROGRAM DESCRIPTION: page 2 More specifically, the program Will: 1. Provide food and shelter in a manner which communicates care and concern. 2. Provide for the basic behavior management of the youth. 3. Create in the group home an appropriate social structure congenial to the needs of the resident youth. 4. Provide each youth with an individualized experience within the group which takes into account his particular needs and his healthy resources as well as those needs which interfere with growth. 5. ProVide remedial education to help the youth maintain or obtain acceptable academic achievements. 6. Provide medical and dental services on a need basis through the Medicaid program. The primary purpose of the planning program will be to identify the most appropriate long-term arrangements for the child and then implement a plan for the child's placement. Such care might be provided either by a regular foster home, a long-term residential care facility or the child's family. The planning program will include the following activities: 1. Psychological evaluation for all youths unless recent (within the last six months) evaluations are available. 2. Ail youths served in this program will be under the custody of and recommended by the Cabarrus County Department of Social Services. The decision for admitting a youth will be the responsibility of Program Manager (or designee) in consultation with the director of the tacility (or designee). Any youth may be admitted except those who are so emotionally handicapped that their inclusion would possibly be harmful to the other youths or the staff. A youth may be admitted to the program at any time through the day or night on any day of the year. He may remain for a maximum of 90 days unless an extension is requested in writing by the Program Manager after consultation with the director of the facility (or designee). Staff of the Temporary Care and planning services will be provided by the N. C. Lutheran Services to Families and Children and the Department of Social Services and through special contractual rate.
