PROGRAM DESCRIPTION: page 2 <br /> <br />More specifically, the program Will: <br /> <br /> 1. Provide food and shelter in a manner which communicates care and concern. <br /> 2. Provide for the basic behavior management of the youth. <br /> 3. Create in the group home an appropriate social structure congenial to the needs <br /> of the resident youth. <br /> 4. Provide each youth with an individualized experience within the group which takes <br /> into account his particular needs and his healthy resources as well as those needs <br /> which interfere with growth. <br /> 5. ProVide remedial education to help the youth maintain or obtain acceptable academic <br /> achievements. <br /> 6. Provide medical and dental services on a need basis through the Medicaid program. <br /> <br />The primary purpose of the planning program will be to identify the most appropriate <br />long-term arrangements for the child and then implement a plan for the child's placement. <br />Such care might be provided either by a regular foster home, a long-term residential care <br />facility or the child's family. <br /> <br />The planning program will include the following activities: <br /> <br /> 1. Psychological evaluation for all youths unless recent (within the last six months) <br /> evaluations are available. <br /> 2. The decision for admitting a youth will be <br />the responsibility of Program Manager (or designee) in consultation with the director of <br />the tacility (or designee). <br /> <br />Any youth may be admitted except those who are so emotionally handicapped that their inclusion <br />would possibly be harmful to the other youths or the staff. A youth may be admitted to the <br />program at any time through the day or night on any day of the year. He may remain for a <br />maximum of 90 days unless an extension is requested in writing by the Program Manager after <br />consultation with the director of the facility (or designee). <br /> <br />Staff of the Temporary Care and planning services will be provided by the N. C. Lutheran <br />Services to Families and Children and the Department of Social Services and through special <br />contractual rate. <br /> <br /> <br />
