Alsoj agencies who . <br />provided services resulting in less then 60% of their base contract amount <br />being claimed within the first two quarters did not receive supplemental <br />funds. The decision' to use the specified methodology for distributing the <br />supplemental funds was based on the following: <br /> <br /> 1) The large numbers and dollar values of the request and the small <br /> ~r~ount actually nvailable~ <br /> 2) The elimination of the historically defined based contract amount as a <br /> limiting factor in an agencyts request and award~ and~ <br /> 3) The desire to provide supplemental awards to as many agencies as <br /> possible without awards being un small as to question their value. <br /> <br /> In accordance with a review of the information your agency uubmitted~ your <br />agency: <br /> <br /> x has been granted supplemental funds in the amount of $4,875. <br /> has not been awarded supplemental funds at this time. <br /> did not apply for a supplemental award. <br /> <br /> For those agencies receiving funds that did not send a signed budget <br />revision with your request; we are enclosing DHS Form 2948E (LHD) or DHS Form <br />2482 (Private). Please complete and sign the budget forms and return them to <br />the Program no later than ~arch 1St 1985. Funds unbudgeted by that datet or <br />without explanation as to extenuatin9 circumstancest will be re-assigned to <br />othe~ a~encies.
