(complete information requested below) <br /> <br /> Please indicate how successful your program has been in achieving the measurable <br /> objectives that were included in last year's CBA Pro§ram Agreement. (Use data <br /> from tile first six months of the fiscal year.) <br /> <br />Hensurable objectives met July l, 1983 - December 31, 1983 <br /> <br />1. 75 per cent referred children served. <br /> 82% as 9 of tile 11 referred were served. 2 children left in home of parent <br /> with services provided as this was in best interest of children. <br /> <br />2. 60 per cent prepared for long-term placement. <br /> 67% as 7 of the 11 admissions were placed successfully in long-term placements. <br /> 2 were discharged after being on runaway status, 1 was placed in training <br /> school, 1 in mental hospital. <br /> <br />3. 50 per cent achieve successful adjustment for six months. <br /> 67% as 6 of the 9 children placed in the first six months of calendar year 1983 <br /> (3ammry - June, 1983) have completed six months follow-up reports and have <br /> achieved successful adjustment for the period.
