ATTAC}~NT B: <br /> <br />Information maintained for effectiveness measurement: <br /> <br /> (1) Successful completion of the prugram will be measured by item <br /> 13 of the CBA Client Tracking Form for School Related Programs. <br /> <br /> (2) Behavioral a~itudieal checklist will be completed by the <br /> classroom teacher at the conclusion of the group counseling <br /> experience as shown by item 15 of the Client Tracking Form. <br /> <br /> (3A) Ail students upon entering the counseling program will be <br /> administered a self-concept test such as Pierce-Harris Self- <br /> concept Scale~ <br /> <br /> (3B) Each student, as he completes the group counseling experiences, <br /> will be administered the same test as on entering the program. <br /> <br /> (4) Based on information obtained from school records and/or interviews <br /> with the guidance counselor, principal, teacher, student, other <br /> agency referral sources - data will be gathered concerning office <br /> visits for disciplinary reasons~ truancy, suspenpions, expulsions, <br /> and court referrals. <br /> <br /> <br />
