NOR~.d CAROLINA CONTRACT FOR ' <br /> }~CKLENBURG COUNTY ~RGENCY SERVICES EQUIP.WJ~NT <br /> <br /> I~HIS AGREE}~;T, made and entered into this __ day of __ ~, 1982, <br />By and between Cabarrus Coun.tv , hereinafter referred to as the <br />County, and the CENT~a_LINA COUNCIL OF GOVE~N~_NTS, hereinafter referred to aa the <br />Council; <br /> <br /> WI T~_UE S SET'~: <br /> <br /> ~,~HEREAS, Cabarrus County desires to improve their emergency <br />medical services program, and has submitted to the Council a proposal] for such <br />improvements;, and <br /> <br /> WIREP, EAS, said proposal was included in the E}[S Area Iii Emerzenc¥ Medical <br />Services System Improvement - 1204(1). application p~epared by the Council for <br />submi.~tal to the U.S. Department of Heal]th ~od Human Services through the N.C. <br />Department of Human Resources~ and <br /> <br /> '~CH~REAS, the Council's application, including the County's proposal, has <br />been approved and subsequently funded; <br /> <br /> NOW THEREFORE, in consideration of the premises and pay~aent of the funds <br />as hereinafter set forth, both the County and the Council do mutually, agree to <br /> .'he fo!!owin~: <br /> <br />A. GENERAL Uiq-DERSTA~D iN GS <br /> <br /> The Council agrees to provide or to cause to be provided chose certain <br /> items of ~!S Communications Equipment set forth in Exhibit A, attached <br /> '' hereto and incorporated herein by reference, <br /> <br /> 2. The Council agrees to pay 75% of the ~otal purchase price of ~hose <br /> certain items sat forth in Exhibit A, provided than under no circum- <br /> stances shall the amount paid by the Council exceed $ S!!4~026,6~ __. <br /> <br /> 3. The County agrees to provide toward the total purchase price an amount <br /> equal to at least 25% of the total purchase price of items set forth <br /> i~n E:dnibit A. <br /> <br /> 4. Paymen~ of funds by the Council as specified i~ this agr~emen~ is <br /> conditioned upon and subject to the Council's receipt of funds allocated <br /> and appropriated for this purpose from the N.C. Department of Human <br /> Resources. <br /> <br /> 5. The County agrees to name as their <br /> official project director for all ma=tars pertaining to this agreement. <br /> <br /> 6. l~ae County agrees to submit in writing to the Council any request fo'r <br /> changes in the aforesaid equipment or budge~ therefor, and, to receive <br /> the approval of the Council prior to implementing any such changes. <br /> <br /> <br />
