A file is <br /> maiatainod by Lutheran Family Services on each child admitted. This file contains the <br /> following forms: <br /> <br /> (1) Child's information sheet completed at admission. <br /> (2) Placemenn Progress Report. This form keeps a running record weekly of what progress <br /> has bee, made toward a plan for this child in leaving ~emporary shelter care. <br /> A weekend activity planning record. <br /> (4) A daily log showing the child's disciplinary actions, DSS contact, personal observation <br /> by staff and co~euts regarding the child's adjustmeat that day. <br /> {5) ~ approach plan which describes the child's problems, needs and approach to meeting <br /> the needs. <br /> (6) A basic clothing list. <br /> (7) Receipts for purchases <br /> (8) Comments from the staff or other information as needed. <br /> <br /> At t)m DSS iaformation is kept in the child's folder 'regarding the date he was placed at <br /> Lutberan Home, each coaference that is held with the child or his family or any outside <br /> agency, all medical, psychological and education conferences and a copy of the termzuation <br /> s~nary completed by Lutheran Family Services. Iu addition, the CBA tracking forms are <br /> kept on each child referred. Contracts for consulting services, copy of liceuse, names and <br /> qualifications of individuals employed as staff aad amount of training as well as proof of <br /> availability of medical assistance is kept on file at Lutheran Services. Copies of letters <br /> of assents are not kept as a screening co~ittee involving DSS, Mental Health. Court <br /> Counseling, law enforcement and school meets approximately each month to review residents <br /> at the shelter care. Coordination with law eoforcement is done through the task force <br /> meetings as well as the screening co~ittee meetings aud averages contact; at least monthly. <br /> A listing of the members of the local advisory board is kept by the task force chairpersou. <br /> The CBA client tracking forms will evaluate the uumber of children referred and those who <br /> were not admitted and why they were rejected. Lutheran Homes will keep a file of the n~ber <br /> of children who ran away from the project facility during this period. <br /> <br /> Tbe DSS will continue to evaluate the numbmr of cbildren wbo are involved in a crisis <br /> situation and compare that number with the aumber of admissions to the Temporary Shelter <br /> Care, Statistics will be kept regarding n~ber of children placed in other facilities who <br /> were in crisi~ situations as well as the n~ber placed i~] Lutheran Home. Lutberan Home <br /> and DSS files will provide information to dec,neat the number of children who were placed <br /> in the Temporary Shelter Care and those who achieve stability and those who did uot. The <br /> DSS will contact each child after he has completed his stay at the Temporary Shelter Care <br /> six months after discharge date to determine if the child continues to reside gracements, <br /> if he has come to the attention of the court, what his school adjustment has been and if <br /> the child's parents, guardiaa or placement per,on agrees that the situation has been stable <br /> for the past six montbs This will be doue by a contact from DSS. <br /> <br /> <br />
