Participate in th~ interdisciplinary team conference on a regular <br /> basis. <br /> Provide direct care or telephone consultation on an e~_rgency <br /> basis to high risk matei~ity patients during nonclinio hours. <br /> d. Provide special t~sting such as ultrasounds, stress and non-stress <br /> tests, and other laboratory tests for the high risk patient at a <br /> cost not higher than the Medicaid allowance. <br /> e. Provide ~spital delivery care at Cabarrus MemOrial Hospital on <br /> a rotating basis to high risk patients. <br /> <br />B. Clinic Setting: <br /> <br /> Cabarrus County Health Department is located four miles from Cabarrus <br /> F~/~orial Hospital and twenty-four miles frc~ the Level III facilities, <br /> Charlotte Memorial Hospital. The high risk maternity clinic will meet <br /> weekly and will serve Cabarrus and Stanly county patients who meet the <br /> State high risk criteria. Care will be provided by an interdisciplinary <br /> team consisting of: Board certified obstetricians, two family nurse <br /> practitioners, two public health nurses (one of whom is a PHN III - <br /> Clinic Coordinator), one c~,u,unity health aide, one MCH nutritionist, <br /> one social worker (who is currently being recruited), and one half-time <br /> nutrition trainee. <br /> <br /> <br />
