To attend special meetings and workshops related to high risk <br /> <br /> Social Worker II <br /> <br /> This position is funded through MCH and approximately 25% of the time <br /> will be spent in the high risk maternity program. This individual will: <br /> <br /> a. Interview patients and/or family ms,be_rs to develop social history <br /> and to determine the nature and ~t of ~tional, envir~rm~ntal, <br /> personal, and/or social problems. <br /> b. Counsel all maternity patients - high risk and rec~]ar - as needed. <br /> c. Counsel teen patients concerned with problem pregnancy, early <br /> responsibilities of parenthood, and problems concern~ family <br /> planning methods. <br /> d. Act as liaison between private physicians and health department, <br /> prenatal clinic, ocmm~u~ity, district, ara regional hospitals, <br /> all other cc~am~nity resources in the realm of social work involvement. <br /> e. Provide ongoing consultation to the ~interdisciplinary team n~_mbers on <br /> the socio-cultural implications of high risk pregnancy using the <br /> post clinic conference as the vehicle for the diss~aination of this <br /> information. <br /> f. Use cc~munity organizational skills to develop various group <br /> activities such as proper parenting, marital relationships, family <br /> relationships, etc. <br /> g. Assist in clinic education as w~ll as school and cc~munity groups. <br /> h. Be responsible for coordinating referral systems to provide continuity <br /> of care and family involvement in decision making. <br /> i. R~fer patients to appropriate resources. <br /> <br />3. Nutrition Trainee: <br /> <br /> This will be a half-time position. The responsibiliti~.s of this position <br /> will include the following: <br /> <br /> a. Foll(~wup 'of high risk maternity patients by making hom~ visits to <br /> help assess the patients' fcod habits and to instruct the patients <br /> in a better dietary pattern. <br /> b. Assist in clinic education such as group classes in prenatal nutrition, <br /> classes in infant feeding, and food preparation demonstrations. <br /> c. Participate as a msmber of the interdisciplinary team in post clinic <br /> <br /> d. Provide adequate documentation of hc~e and clinic visits. <br /> <br /> This person will be directly responsible to the nutritionist. <br /> <br /> <br />
