NOTES ON IN-HOME SERVICE COORDINATION - CENTRALINA COG <br /> <br /> 1. In order to prepare the required plan and memorandum of understanding, <br /> you must prepare an Inventory of In-Home Services. This will be filled <br /> out by each agency which does (or intends) co provide: <br /> <br /> Adult Day Care <br /> Chore Services <br /> Homemaker Servmces <br /> Housing and Home Improvement <br /> Preparation and Delivery of Meals <br /> Home Health Care <br /> <br /> COG has prepared a set of forms (along with definitions and instructions) <br /> to capture the required information, and will supply copies upon request. <br /> <br />2. The inventory requires an estimate of number ~n need for each service. <br /> COG has %ome data from the statewide Title ~ household survey for most <br /> of these services (except home health), which it will make available <br /> upon request. <br /> <br />3. The State Office on Aging has developed a sample Flan and a sample <br /> memorandum of anderstandin~, which meet the minimum requmrements. <br /> Included with each of these is a checklist of what is required to pro- <br /> duce them. COG will provide copies of.these upon request. <br /> <br />4. Timin~ is becoming critical. If you have noc already started, the <br /> following timetable is suggested, to meet the deadline. <br /> <br /> (1) Meeting of agencies - form~orking committee 1st week - February <br /> (2) Develop inventory forms and send out 1st week - February <br /> (3) Agencies fill Cut inventory 2nd & 3rd week - <br /> February <br /> (4) Compile total inventory and <br /> Joint agency review/finalize 4th week - February <br /> (5) Draft p~an and memorandum 1st & 2nd week -~March <br /> (6) County commission approval 3rd & 4th week - March <br /> (7) D~e to DHR, Division of plans and <br /> operations March 31, 1980 <br /> <br /> <br />
