IR-HOME SERVICES COORDINATION <br /> <br /> MEMO~RNDUM OF UNDERSTANDING <br /> <br /> I. The purpose of this memorandum is: (1) to specify <br /> the responsibilities and activities of local agencies <br /> that are participating in the delivery of in-home <br /> services; (2) to ensure that in-home services are <br /> being coordinated locally; and ~(3) to describe the <br /> local coordination process for[the delivery of <br /> in-home services. <br /> <br /> II. The parties have agreed to participate in the <br /> development of' a coordinated in-home services plan. <br /> That plan, when completed~ is specifically made a <br /> part of this Memorandum of Understanding. <br /> <br />III. ,The following paragraphs ~escribe the local coordi- <br /> nation process and mechanism that was Used in the <br /> development of the local in-home services plan ahd <br /> also describes the process and mechanism for <br /> implementing that plan: . <br /> <br /> IV. This agreement, when accepted by all pa~ties, will <br /> be gffective until replaced by another memorandum <br /> of understanding or until specifically rescinded. <br /> This memorandum may-be amended by mutual consent <br /> of all'parties. A party may terminate their <br /> participation under this Memorandum of Understanding <br /> 'by giving thikty (30) days written notice to the <br /> other parties. <br /> <br /> <br />
