GOAL$~' <br /> <br /> To develop and conduct extensive educational programs for the generai <br /> public on diabetes and glaucoma~ <br /> <br />2. To establish a program in Cabarrus County for screening persons over 35 <br /> years of age for early signs of diabetes or'glaucoma. <br /> <br />3, To prevent disability from blindness due to glaucoma. <br /> <br /> To reduce the cost of health care by early detection, referral and <br /> treatment of diabetes or glaucoma on outpatient basis and prevention of <br /> hospitalization. <br /> <br />5. 7o determine cost of services on a unit basis (e.g., cost of screening <br /> visit, counseling session, etc.) through data collection by supervisory <br /> nurse, <br /> <br /> To establish a register of persons with diabetes and provide all necessary <br /> follow-up procedures ordered by patient's physicians. <br /> <br /> To establish program for teaching diabetics and providing familial <br /> counseling. <br /> <br />8. To coordinate program services with opthalmologists, optometrists and <br /> health department services, <br /> <br />9. To coordinate screening and educational activities of community groups. <br /> <br />O~JECTIVES <br /> <br />I. Implement comprehensive educational programs to motivate those diabetics <br /> and glaucoma patients to be screened. <br /> <br />2. Screen 50% of Cabarrus County citizens at risk for diabetes or glaucoma <br /> in first year. (At risk is defined as over 35 for glaucoma and pregnant, <br /> obese or having a diabetic parent.) <br /> <br /> 10 <br /> <br /> <br />
