,G~auoo~a will be held af the Carolina Hall, Logan School and o~her ~ommunity <br /> sites such as the Life Center, a day care center providing recrealional <br /> and social activities for senior cilizens. Regular screening clinics will <br /> take place at the health departmenl, however, industrial sites and satellite <br /> clinics will be utilized in conjunclion wilh the Lion~ Clubs and olher <br /> civic organizations. <br /> <br />Objective 3 - Maternity patients are routinely screened for diabetes. Clients <br /> determined to be at risk will be referred to High Risk Maternity Clinic and <br /> to the public health nutritionisf for counseling. <br /> <br />Objective 4 - The project nurse will establish necessary contacts to develop <br /> volunteer resources and coordinate their efforts. Key contacts include <br /> Lions Clubs, Diabetes Association, Medical and Optometric Societies and <br /> other support groups. Through these contacts, an effective network <br /> providing public information concerning the project will be established. <br /> The role of the volunteer Is an integral part of the project. Training of <br /> these volunteers will be done by the project nurse. <br /> <br />Objective 5 - The necessary records will be maintained to account for numbers <br /> of persons served, numbers and kinds of services rendered, monitoring end <br /> long-term follow-up of patients, and outcome measurements such as degree <br /> of sight lost, days of hospitalization experienced, etc. This data will <br /> serve in a determination of cost of services on a unit basis. In addition, <br /> this information will be used in preparation of required reports for N.C. <br /> Division of Health Services. <br /> <br />Objective 6 - Local ophthalmologists will offer consultation regarding problems <br /> with screening, referral or follow-ap of persons seen in a screening clinic. <br /> Glaucoma and/or diabetes referrals will be made Io Ihe pruclitioner d~sign.~led <br /> by the person screened.
