Tomlinson, Jr., to the Kannapolis Street Planning Board for a <br />one-year term. <br /> UPON MOTION of Chairman Cook, seconded by Commissioner Lentz and unanimously <br />carried, the Board designated April 14, 1980 at 10:00 A.M. as the date and time <br />fdr a public hearing for the following road name changes. <br /> <br />Old Name <br />Booker Avenue <br />Unnamed Road <br />Unnamed Road <br />Unnamed Road <br />Woodgreen Drive <br /> <br />New Name <br />Onyx Avenue <br />Asbury Road <br />Biltmore Drive <br />Shalimar La~-e <br />Fringewood Drive <br /> <br /> UPON MOTION of Commissioner Lentz, seconded by Commissioner Bonds and <br />unanimously carried, the Board moved to amend the budget of the Department <br />of Social Services to accept an additional $6,000.00 in State funds for <br />Family Planning Services. <br /> UPON MOTION of Commissioner Milloway, seconded by Commissioner Lentz and <br />unanimously carried, the Board adopted the following In-Home Services <br />Coordination Memorandum of Understanding approving the Local Coordination of <br />In-Home Services Plan as required by the Department of Human Resources. <br /> <br />IN-HOME SERVICES COORDINATION <br />MEMORANDUM OF UNDERSTANDING <br /> <br />The purpose of this memorandum is: (1) to specify the responsibilities <br />and activities of local agencies that are participating in the delivery <br />of in-home services; (2) to ensure that in-home services are being <br />coordinated locally; and (3) to describe the local coordination process <br />for the delivery of in-home services. <br /> <br />II. <br /> <br />The parties have agreed to participate in the development of a coordinated <br />in-home serv~esplan. That plan, when completed, is specifically made a <br />part of this Memorandum of Understanding. <br /> <br />III · <br /> <br />The following paragraphs describe the local coordination process and <br />mechanism that was used in the development of the local in-home services <br />plan and also describes the process and mechanism for implementing <br />that plan: <br /> <br />IV. <br /> <br />This agreement, when accepted by all parties, will be effective until <br />replaced by another memorandum of understanding or until specifically <br />rescinded. This memorandum may be amended by mutual consent of all <br />parties. A party may terminate their participation under this Memorandum <br />of Understanding by giving thirty (30) days written notice to the other <br />parties. <br /> <br />V.
