120 <br /> <br />responder unless that person is affiliated with a certified first <br />responder agency and holds a currently valid certificate to operate <br />as a first responder issued by the Cabarrus County Office of <br />Emergency Services and approved by the agency's Fire Chief or <br />designee. <br /> <br />Section 3. Application and Granting Process for Certification as a First <br /> Responder or an Individual First Responder. <br /> <br />Application for certificate to operate as a first responder agency <br />in Cabarrus County by an applicant agency shall be filed with the <br />Cabarrus County Office of Emergency Services. This application <br />shall be made upon such forms as may be prepared by the Cabarrus <br />County Office of Emergency Services and shall contain: <br /> <br />A. The name and address of the applicant agency. <br />B. The trade name of the applicant agency under which its business <br /> is conducted. <br />C. The fire district in which it will operate. <br />D. A plan describing how the agency will respond when dispatched <br /> on first responder calls. <br />E. Any information Cabarrus County Office of Emergency Services <br /> shall deem reasonably necessary for a fair determination of the <br /> capacity of the applicant agency to provide first responder <br /> service in the service area in accordance with Cabarrus County <br /> requirements. <br /> <br />Me <br /> <br />Application for certificate to function as an individual first <br />responder in Cabarrus County by an applicant individual shall be <br />filed with the Cabarrus County Office of Emergency Services by the <br />applicant Fire Chief or his/her designee with proper approval from <br />the corporate Board of Directors. This application shall be <br />made upon such forms as may be prepared by Cabarrus County <br />Emergency Services and shall contain: <br /> <br />The name, age, social security number, driver's license number, <br />and address of the applicant individual. <br />The dates, location, and instructor of a successfully completed <br />basic first responder training course by the applicant <br />individual. <br />The expiration date of American Heart Association or American <br />Red Cross Cardiopulmonary Resuscitation certification of the <br />applicant individual. <br />The name of the first responder agency with which the applicant <br />individual-is to be affiliated. <br /> <br />Upon receipt of an application approved by the Advisory board, a <br />certificate to operate as a first responder agency in Cabarrus <br />County, the Cabarrus County Office of the Emergency Services will <br />forward such applications to the Cabarrus County Board of <br />Commissioners for their approval. <br /> <br />The Cabarrus County Board of Commissioners shall approve or <br />disapprove all applications for first responder agency status and <br />grant certificates to operate as a first responder agency in <br />Cabarrus County. <br /> <br />0 <br /> <br />Upon receipt of application for certificate to operate as an <br />individual first responder in Cabarrus County, the Director of <br />Emergency Services or his designated representative will approve or <br />disapprove all applications based on criteria in Section 5 of this <br />document. <br /> <br />Upon approval to operate as an individual first responder in <br />Cabarrus County, a certificate and pocket card will be issued by <br />Cabarrus County Office of Emergency Services. <br /> <br />SECTION 4: Terms of Certification as a First Responder Agency and <br /> Individual Responder. <br /> <br /> <br />
