S. Department of <br />Agriculture Farm Service Agency. The rental term will be for a period of <br />five years with an annual rent of $29,280.00 beginning 2006 and adjusted <br />annually based on the CPI from the previous December 31 each year through <br />2011. The total anticipated revenue for the five-year period will be at <br />least $146,400.00. <br /> <br />UPON MOTION of Commissioner Privette, seconded by Commissioner <br />Carpenter and unanimously carried, the Board approved the U.S. Department of <br />Agriculture Farm Service Agency office space lease agreement for a term of <br />five years and authorized the County Manager to execute the lease agreement. <br /> <br />(F-22) Employee Health Clinic - Contract with NorthEast Medical Center <br /> <br />Cabarrus County received five responses from the Request for Proposals <br />(RFP) for staffing the Employee Health Clinic. The five responses include <br />HealthSTAT, Carolinas HealthCare System, Cabarrus Health Alliance, Cabarrus <br />Family Physicians and NorthEast Medical Center (NEMC). After reviewing each <br />proposal and interviewing each agency, staff recommends NorthEast Medical <br />Center. <br /> <br />UPON MOTION of Commissioner Privette, seconded by Commissioner <br />Carpenter and unanimously carried, the Board approved the selection of NEMC <br />as the medical provider of the Employee Health Clinic for three years (based <br />on 40 hours per week: year 1 - $126,500.00; year 2 - $136,164.60; and year 3 <br />_ $147,057.76); and directed the County Manager and County Attorney to draft <br />and execute the contract with NEMC. <br /> <br />(F-23) NC Department of Transportation Maintenance/Right-of-Way Abandonment <br />for a Portion of Macedonia Church Road (SR 1620) <br /> <br />Property owners (Craven and Cindy Mabrey and Thelma Collins) at the <br />western end of Macedonia Church Road (adjacent to Lake Howell) are requesting <br />abandonment of the State maintenance and public right-of-way for a portion <br />(0.10 miles) of Macedonia Church Road (from Macedonia Church Road Extension <br />to the end of maintenance). After the abandonment of State maintenance is <br />approved by the N. C. Department of Transportation, the Board will be asked <br />to consider the abandonment of right-of-way. <br /> <br />UPON <br />Carpenter <br />Resolution: <br /> <br />MOTION of Commissioner <br />and unanimously carried, <br /> <br />Privette, seconded by Commissioner <br />the Board adopted the following <br /> <br />Resolution No. 2006-17 <br /> <br />RESOLUTION EXPRESSING SUPPORT OF <br />THE CABARRUS COUNTY BOARD OF COMMISSIONERS FOR <br />NORTH CAROLINA DEPARTMENT OF TRANSPORTATION <br />TO REVIEW ABANDONMENT AND CLOSURE OF A PORTION OF <br />STATE ROAD NUMBER 1620 <br />ALSO KNOWN AS MACEDONIA CHURCH ROAD <br /> <br />WHEREAS, North Carolina State Road 1620 in Cabarrus County is known as <br />Macedonia Church Road, being a paved road approximately 1.3 miles in length, <br />with the last .10 miles being the portion requested to be abandoned. <br /> <br />WHEREAS, the attached petition has been filed with the Board of County <br />Commissioners of the County of Cabarrus requesting that the above described <br />
