<br /> UPON MOTION of Commissioner Privette, seconded by Commissioner Carpenter
<br />and unanimously carried, the Board approved the revised Home and Community Care
<br />Block Grant Funding Plan for FY 2004 and the budget amendment reflecting an
<br />increase of $5,247.00 and grant funds of $23,920.00 from the Robert Wood Johnson
<br />Foundation for the 30 Minutes for Life Program through the University of Illinois
<br />for the Senior Center Wellness Program.

<br />AMOUNT: $29,167.00
<br />TYPE OF ADJUSTMENT: Supplemental Request
<br />PURPOSE OF REQUEST: Department of Aging budget amendment to reflect an increase
<br />of $5,247 in the Home and Community Care Block Grant funding plan for FY'04 due
<br />to receiving our final allocation based on the approved state budget. Our
<br />original funding plan was based on estimated figures pending approval of the
<br />state budget. We are also appropriating $23,920 in this request for additional
<br />grant funding from the University of Illinois (Robert Wood Johnson Foundation
<br />funds) for our Wellness program. We will receive one-half of grant funding
<br />($11,960) in October 2003 and the remaining funding in March 2004.
