259 <br /> <br />NEW BUSINESS CONSENT <br /> <br /> UPON MOTION of Commissioner Hamby, seconded by Commissioner Barnhart and <br />unanimously carried, the Board authorized the Chairman to execute a Three Party <br />Right of Way Encroachment Agreement on Primary and Secondary System for the <br />installation of a water line on State Road 1274 by Mr. and Mrs. Carl Baler. <br /> UPON MOTION of Commissioner Hamby, seconded by Commissioner Barnhart and <br />unanimously carried, the Board approved the following Annual Agreement between <br />Cabarrus County and Cabarrus County Health Department and Hospice of Cabarrus <br />County, Inc. <br /> <br /> AGREEMENT BETWEEN <br />Cabarrus County and Cabarrus County Health Department <br />and <br /> HOSPICE OF CABARRUS COUNTY, INC. <br /> <br /> This Agreement is entered into as of this 1st day of July, 1991, by and <br />between Cabarrus County ("County"), Cabarrus County Health Department ("Health <br />Department"), and Hospice of Cabarrus County, Inc. ("Hospice"). <br /> WHEREAS, Hospice is engaged in providing interdisciplinary care and <br />treatment of terminally ill patients and their families in order to allow these <br />families to continue life with minimal disruption; and <br /> WHEREAS, it is in the mutual best interest of the parties, the community <br />and the patients of Hospice for the specific services as described herein to be <br />provided; and <br /> WHEREAS, County and Health Department have identified the need for Hospice <br />services to fulfill demands and utilize existing resources in Cabarrus County; <br />and <br /> WHEREAS, Hospice agrees to assume the responsibility for providing Hospice <br />services in compliance with N.C. state licensing laws, under both the applicable <br />policies of Hospice and the applicable Medicare rules and regulations; and <br />within the scope and limitations set forth in the interdisciplinary care plan <br />which is established by Hospice and attending physicians; and <br /> WHEREAS, the supervision of services will be provided by the <br />interdisciplinary team of Hospice and shall be subject to review by the Hospice <br />Patient Care Committee. <br /> IN CONSIDERATION of the mutual covenants and agreements set forth herein, <br />the parties hereby agree ~as follows: <br /> DEFINITIONS <br />Hospice patient: An individual who has been certified as terminally ill by <br />their physician and is receiving care from Hospice of Cabarrus County, Inc. <br />Attending physician: A doctor of medicine who is identified by the individual, <br />at the time he or she chooses to receive Hospice care, as having the most <br />significant role in determination and delivery of the individual's medical care. <br />Hospice employee: A person, working under the jurisdiction of the Hospice, <br />either salaried or volunteer. <br />Patient Care Coordinator: A' registered nurse designated by the Hospice to <br />coordinate the implementation of the care plan for each Hospice patient. <br />~ospice Interdisciplinary team: A group of Hospice employees and volunteers, <br />consisting at a minimum of a doctor of medicine, a registered nurse and a social <br />worker who provideor supervise the care and services offered by the Hospice. <br />Care Plan: A written plan prepared for each Hospice patient, containing an <br />assessment of the~Hospice patient's needs, identification of services to be <br />provided, and a detailed description of the scope and frequency of services <br />needed to meet the patient's and family's needs. <br /> SERVICES <br />Hospice will provide 24 hour, seven day a week on-call hospice care to Hospice <br />patients and their families utilizing a multi-disciplinary team approach. <br />Hospice care is palliative rather than curative, and Hospice seeks to meet its <br />patient's physical, psYchological, social, and spiritual needs in a manner that <br />promotes the Hospice philosophy of affirming life. <br /> COMPENSATION <br />County and Health Department shall pay Hospice $5,592.00 for rendering 200 <br />hospice patient~care days to appropriate residents of Cabarrus County within the <br /> <br /> <br />
