386 <br /> <br />The Provider agrees to perform the following services: <br />a. Conduct an extensive search for a qualified candidate for the <br />OB/Gyn Position including contact with approved residency <br />training programs in OB/Gyn <br />b. Screen candidates according to Health Department' criteria <br />c. Interview appropriate candidates and market Health Department <br /> position in most favorable terms <br />d. Consult with Dr. Allen Dobson, Dr. Vincent Arey, and Fred <br /> Pilkington on specific requirements of position <br />e. Provide in writing three (3) references on candidate <br />f. Set up telephone and face-to-face interviews <br />g. Guarantee qualified candidate no later than July, 1991 <br />h. Provide progress reports on advertisements results, interviews <br /> and reference checks <br />i. Periodically meet with Health Department officials to review <br /> program of recruitment <br /> <br />The Cabarrus County Health Department agrees to the following: <br /> a. To pay Provider as follows: <br /> (1) $ 3,000.00 Retainer before July 1, 1990 <br /> (2) 6,000.00 Within one week after face-to-face interview <br /> (3) 9,000,00 Within one week of signing of employment <br /> contract by candidate and Cabarrus County <br /> Health Department <br /> $18,000.00 Total Fee <br />b. To interview any candidate presented by the Provider who meets <br /> desired qualifications so specified by the employer within 48' <br /> hours. <br /> c. To reimburse Provider on an hourly rate of $100.00 plus travel <br /> expenses not to exceed $2,500.00 per year for practice <br /> consultations and visits to candidates prior to hiring <br />d. To offer a second OB/Gyn physician placement at a total fee <br /> not to exceed $17,000.00 with no additional retainer. Lyons, Chairman of the Community Based Alternatives (GBA) Task <br />Force presented the Program grant application proposals for CBA funding for <br />Fiscal Year 1990-1991 as follows: Psychological Services to Juvenile <br />Courts/Relapse Prevention Services ($21,000.00), Home-Based Services <br />($24,865.00), and the Temporary Shelter Care Project ($95,900.00). He reported <br />that the Task Force was requesting that the County supplement the CBA funds with <br />an appropriation of $7,163.00. <br /> <br /> <br />
