553 <br /> <br />NEW BUSINESS CONSENT <br /> <br /> UPON MOTION of Chairman Lentz, seconded by Commissioner Hamby with <br />Chairman Lentz and Commissioners Hamby, Moss, and Simmons voting for, the Board <br />adopted nunc pro tune the "Contract for Respite Care Adult Day Care Facility" <br />between First Assembly Adult Day Care (the "Center") and the Department of Aging <br />(the "Purchaser") for adult day care services. The contract for the period July <br />1, 1990 through June 30, 1991 provided for the purchase of up to 576 unit hours <br />with no more than 48 hours per client per month within the contract period. The <br />purchase and delivery price for all units of service was set at a unit price of <br />$3.25 per hour with the responsibilities outlined in the contract. The contract <br />is on file with the Clerk to the Board and is hereby included by reference as <br />a part of the minutes. <br /> UPON MOTION of Chairman Lentz, seconded by Commissioner Hamby with <br />Chairman Lentz and Commissioners Hamby, Moss, and Simmons voting for, the Board <br />adopted the following amendment to the "Contract for Respite Care Adult Day Care <br />Facility" between First Assembly Adult Day Care and the Department of Aging for <br />adult day care services. <br /> <br />CABARRUS COUNTY <br /> <br />AMENDMENT TO CONTRACT <br /> <br /> This Amendment, entered into this 13th day of September, 1990, <br />by and between Cabarrus County Department of Aging (hereinafter <br />called the "Purchaser") and First Assembly Adult Day Care Center for <br />adult day care services (hereinafter called the "Center"). <br /> <br />WITNESSETH THAT: <br /> <br /> WHEREAS, the Purchaser and the Center into that certain <br />agreement dated July 1, 1990 pursuant to which the Center was to <br />provide day care services for those persons who qualify for respite <br />care as defined in the contract, and <br /> WHEREAS, certain changes need to be made in purchase and <br />delivery price for all units of service ordered by the Purchaser; <br /> NOW, THEREFORE, it is agreed that the aforesaid contract be <br />and it is hereby amended in the following respects: <br /> Item number five (5) is amended by deleting the unit price of <br />$3.25 per hour and inserting in lieu thereof the following unit <br />price of $18.00 per day (with a six hour minimum stay) shall be the <br />purchase and delivery price for all units of service ordered by the <br />Purchaser. Monthly request for payment by the Center shall include <br />but not be limited to an individual participant summary consisting <br />of the dates, and total hours, not to exceed 42 hours per month and <br />the total amount owed by Purchaser. <br /> Ail Amendments shall be effective as of July 1, 1990 except <br />where and to the extent specifically provided to the contrary in the <br />Contract and all Amendments thereto. <br /> In witness whereof this Amendment is executed by the parties <br />the day and year first above written. <br /> ATTEST <br />County: Cabarrus <br />Signature: /s/ James W.
