554 The purchase and delivery price for all units of service ordered by the Purchaser shall not exceed a cost of $26.00 per day. This is the Center's medicaid reimbursable rate. An unit of service shall be $4.33 per hour based on a six (6) hour respite stay. In the event that more or less than six (6) hours are contracted by the participant and the Purchaser, the number of hours requested shall be divided by $26.00 for the hourly unit rate thereby not exceeding the $26.00 per day maximum. The unit price shall include the responsibility outlined in this contract. The contract is on file with the Clerk to the Board and is hereby included by reference as a part of the minutes. UPON MOTION of Chairman Lentz, seconded by Commissioner Hamby with Chairman Lentz and Commissioners Hamby, Moss, and Simmons voting for, the Board adopted the following amendment to the contract between First Assembly Adult Day Care and the Department of Aging for adult day health services. CABARRUS COUNTY AMENDMENT TO CONTRACT This Amendment, entered into this 13th day of September, 1990, by Cabarrus County Department of Aging (hereinafter called the "Purchaser") and First Assembly Adult Day Care Center for adult day health services (hereinafter called the "Center"). WITNESSETH THAT WHEREAS, the Purchaser and the Center into that certain agreement dated July 1, 1990 pursuant to which the Center was to provide day health services for those persons who qualify for respite care as defined in the contract, and WHEREAS, certain changes need to be made in purchase and delivery price for all units of service ordered by the Purchaser; NOW, THEREFORE, it is agreed that the aforesaid contract be and it is hereby amended in the following respects: Item number five (5) is amended by deleting the unit price of $4.33 per hour based on a six (6) hour respite stay and also deleting the pro-rate (in the event that more or less than six (6) hours are contracted by the participant and the Purchaser, the number of hours requested shall be divided by $26.00 for the hourly unit rate thereby not exceeding the $26.00 per day maximum) and inserting in lieu thereof the following unit price of $26.00 per day (with a six (6) hour minimum stay) shall be the purchase and delivery price for all units of service ordered by the Purchaser. Monthly request for payment by the Center shall include but not be limited to an individual summary consisting of the dates and total hours not to exceed 42 hours per month and the total amount owed by the Purchaser. Ail Amendments shall be effective as of July 1, 1990 except where and to the extent specifically provided to the contrary in the Contract and all Amendments thereto.
