(Harvest) appreciates this opportunity to submit our cost estimate for <br />professional services at the above referenced site. Harvest Is a licensed General Contractor in North Carolina, <br />South Carolina, and an NCDOT SBE. Please do not hesitate to contact us for any additional information about <br />our qualifications and/or work experience. <br />BACKGROUND <br />Our proposal is based on plans dated original plans dated 8-6-2003, and current existing conditions provided from <br />DMP. Also our site visit and inspection on 4-6-2016, <br />SCOPE OF SERVICES <br />The following outlines the features of our proposed work plan for the restoration of the existing pond located on the <br />backside of the existing school adjacent to the track. <br />POND RESTORATION <br />• We have included cleaning up the slopes and bottom of the pond. <br />• We will then bring in exported material from the New Kannapolis Middle School site. Bring the bottom of <br />the dry pond up to the original elevation, using a vibratory roller to compact in place. Then building the <br />slopes back up to original condition, track in with a dozer. <br />• Replacement of the 42" CMP Riser with trash rack/anti vortex (Aluminum & CMP option, see below) <br />• We will then stabilize the slopes with seed, fertilizer, & straw matting. <br />We will install a new rip rap donut ring (with fabric) around the outlet riser structure. <br />We will place rip rap (with fabric) at all the end sections that discharge to the pond. <br />The final surveying of the pond will be the owner responsibility. <br />LABOR $5,800.00 <br />MATERIALS $2875.50— <br />EQUIPMENT <br />287550—EQUIPMENT $12,265.00 <br />SUB -CONTRACTOR $3,540.00 <br />PO Box 548 Indian Trail, North Carolina 28.79 Phonc (704) 553-0717 Fax: (704) 553-0750 Emalt:bmyers@Ma Menviro—nta%. <br />Attachment number 2 \n <br />F-7 Page 103 <br />
