Cause: Training <br />Management's Response: This missing documentation related to services performed by a <br />provider that was unaware of the required documentation standards. A corrective plan of action <br />that included training was required of the provider. <br />Name of Contact Person: Renee Snipes, Finance Officer <br />Corrective Action: A corrective action plan was required of the contract provider that requires <br />training and monitoring by quality review staff. <br />Proposed Completion Date: February 1, 2007 <br />Summary Schedule of Prior Year Audit Findings <br />Finding 2005-1 <br />Condition: Out of 40 Medicaid patient charts tested, 1 was missing a service plan with a date of <br />service. <br />Recommendation: We recommended ongoing training and review of patient files for complete <br />documentation. <br />Current Status: While management instituted continued training for all staff and also has an <br />internal audit function to review medical records, isolated errors in documentation continue at <br />the contract provider level. While these errors appear to be related to human error and also <br />appear to be isolated occurrences, the Authority provides ongoing training and monitoring to <br />minimize errors in patient files. <br />(38) <br />G - `~ <br />
