12 <br />4. Transportation <br />a. Description: Transportation for job seeking and work is an absolute necessity <br />for Work First recipients. If they do not have access to a vehicle, the Work <br />First program will pay or reimburse them once they obtain a job, and are <br />stabilized on that job, which has been determined to be 30 days from the date <br />of employment or up to 90 days if recipient is receiving Job Bonus benefits. <br />In some instances, the Work First recipient may have a vehicle, and <br />reimbursement can be made at a rate that will be set in January 2007. As in <br />provided transportation, reimbursement will be made for 30 days from the <br />date of employment. Efforts will be made to encourage and provide <br />incentives to car pool. <br />b. Provision: Presently, Cabarrus County has a county run transportation <br />system, Cabarrus County Transport Service (CCTS), as well as a public bus <br />system, C-K Rider. Taxis are utilized as well as the vans when necessary. <br />a Location: Transportation services are provided presently to jobs, both in <br />county and out of county, within a reasonable distance. <br />5. Health Support <br />a. Description: Health Support services at Cabarrus County DSS are provided <br />by the Work First social workers. Social workers assist Work First recipients <br />in identifying and recognizing physical and mental health-related needs, cope <br />with functioning limits, provide counseling, and securing admission to <br />medical, drug treatment, and other health-related facilities. <br />b. Provision: Assessment is done by the Work First social worker. Various <br />health facilities and providers in the community provide services. <br />c. Location: The assessment for health support is completed by the Work First <br />social workers, and services take place at various locations. <br />6. In-Home Aide <br />a. Description: There have been instances where social work staff has worked <br />with Work First recipients who have needed the "hands on" assistance of a <br />paraprofessional in-home aide to provide guidance in home management, <br />personal care. With the limits on CAP and Personal Care Services, there is <br />the assumption that the work of an in-home aide may become increasingly <br />needed in the future to allow elderly or disabled family member in the home <br />to remain there while the Work First recipient participates in work-related <br />activities. <br />b. Provision: This service would be provided in the home of the Work First <br />recipient by a qualified in-home aide, paid for from Work First funds. <br />Location: The home of the Work First recipient would be the service <br />location for in-home aide services. <br />7. Personal and Family Counseling <br />a.
