<br />recommending payment to the County; processing requests for information; preparing design <br />clarifications; reviewing shop drawings; processing change orders (as applicable) and time <br />extension requests, and project closeout. Construction Quality Assurance (CQA) testing will be <br />provided by COM during construction. Survey costs are to be the contractor's responsibility, <br />along with the preparation of record drawings. <br /> <br />It is anticipated that COM will conduct six site visits during the course of the project, to include <br />one site visit for pre-bidding and one site visit just prior to the start of construction for the <br />preconstruction conference. The remaining four site visits will be to review activities during <br />construction for conformance with the Contract Documents. Two additional site visits will be <br />for Substantial and Final Completion inspections. Additional site visits conducted by COM at <br />the County's request or as needed during construction, will be considered out-of-scope. COM <br />will therefore be entitled to additional compensation should additional site visits be necessary. <br /> <br />Contractor costs are not included in this agreement. This agreement will be amended to iriclude <br />contractor costs after receipt of informal bids and the selection of a contractor. Once the <br />contractor is selected construction costs will be identified and included in an amendment to the <br />County, at which time COM will subcontract with the selected contractor and serve as General <br />Contractor for the C&D Landfill expansion project. <br /> <br />CQA testing as stated above will be provided by COM during construction. Similar to contractor <br />costs, the costs for CQA testing have not been included in this agreement. The costs for CQA <br />testing will be included along with contractor costs in an amendment to this agreement. Lab <br />testing (as needed) to determine the suitability of the onsite soils for berm construction, will also <br />be included in an amendment to this agreement. <br /> <br />The County has identified the need to establish a new borrow source on landfill property, in <br />closer proximity to the new C&D Landfill expansion. A potential site has been identified. This <br />amendment does not include a borrow source investigation to confirm the quantity or suitability <br />of the borrow material. These services will be covered under a separate amendment as <br />requested by Cabarrus County. <br /> <br />TASK 5 - PERMIT TO OPERATE APPLICATION <br /> <br />Following the completion of construction, COM will prepare and submit five (5) copies of a <br />preliminary letter report to serve as the Permit to Operate application for Phase I of the new <br />C&DLandfill expansion. The report shall certify to the expansion being constructed in general <br />accordance with the approved Drawings and NCOENR regulatory requirements. The letter <br />report will include any CQA testing performed by COM and as-built surveys to be provided by <br />the Contractor. Because of the phased approach to construction, COM will submit a final letter <br />report to serve as the Permit to Operate Application for combined Phases I and II. COM will <br />arrange for a site visit by the NCDENR SWS to coincide with the final completion inspection to <br />expedite approval of the Permit to Operate. <br /> <br />Page 4 of 6 <br /> <br />F-cr <br />
