<br /> <br />COMPONENT NARRATIVE CONTINUED (attach for each component) <br />(3) Describe the s ecific referral, screening, admission and termination rocedures. <br />Funding for client services will be covered by the JCPC grant; Referrals can be made through the Substance Abuse <br />Clinician on site at the court house. The Substance Abuse-Human Services Clinician will conduct appropriate clinical <br />evaluations for all direct referrals and coordinate additional treatment efforts at DAYMARK if the client requests such <br />services. The Substance Abuse Clinician will assist the family in understanding how to access other service providers <br />available to them. Terminations will be based on the completion of services, compliance or noncompliance with <br />services offered, or referral to other service providers outside the DA YMARK Center. This will be coordinated with the <br />c1inican and the court counselor. <br /> <br />(4) Describe imelementation to include: <br />a) The primary and supplemental services provided, if applicable: <br />Services will be available as needed. Staff Psychological Associate-Human Services Clinician will provide weekly <br />appointments for assessing referrals. They will communicate with the Court Counselor regarding the most beneficial <br />contact schedule. <br /> <br />b) The means of working with youth and families: <br />A psychological evaluation shall be conducted for each appropriate referral; this evaluation is a clinical interview in <br />which the psychological associate considers emotional, developmental, social and environmental issues that may <br />contribute to the presenting problem. Where therapeutically indicated, testingipsychological assessment may be <br />proved. Based on these findings, treatment recommendations and referrals will be made. Actual delivery of services <br />will be outside the the scope of this grant; however, DA YMARK staff will be available for services including: individual, <br />family and group therapy; anger management; substance abuse treatment; Intensive Family Preservation Services; <br />psychiatric consultations and medication assessments. <br /> <br />c) The daily activities: <br />Services will be available as needed.
