Project Challenge provides diversion, level <br />I, and level II services to the Juvenile Court. 3) Once the referral has been made, an intake process is performed. <br />This process includes completing appropriate applications, medical and emergency contact information and all <br />pertinent program related information. At this time both the participant and the parenVguardian is informed of the roll <br />Project Challenge plays and what is expected from both the child and guardian. 4) Once the participant has fulfilled <br />thier obligation to the juvenile court and completed all required componets of the program, the participant is terminated <br />from Project Challenge and is reported to DJJ through the client tracking process. <br /> <br />4) Describe implementation to include: <br />a) The primary and supplemental services provided, if applicable: <br />Project Challenge's primary service is Restitution. <br /> <br />b) The means of working with youth and families: <br />Staff/participant interactions occur during community service and wilderness trip activities. Program Coordinators <br />inform families'of participant progress and rely on familial input to evaluate participant's success at home. Each youth <br />has an individual program plan that describes the goals and time line with which the youth must comply. <br /> <br />c) The daily activities: <br />Daily program activities include performing community service at a variety of sites. These include nursing homes, food <br />pantries, animal shelters, parks and recreation departments, literacy councils, civic agencies and individuals in need <br />within a community. During the school year, these activities take place after school hours, and on Saturdays.
