<br />complete a written monthly progress report for each case. These progress reports are now forwarded to DJJ. If <br />goals are not being milt, different interventions must be tried. As families develop more appropriate and <br />successful methods of dealing with problems, the Home Based counselor will congratulate the family on <br />progress and move th~ family towards less reliance on the Home Based worker. When families appear to be <br />close to reaching thei~ goals, a target date for termination is set. A closing home visit is scheduled and the <br />progress of the case i$ reviewed. If there are still areas of concem, other resources are recommended. The <br />family is informed thaUhey can call the counselor after services terminate for support or assistance as needed. <br />They are also inform~ that follow-up contacts will be made quarterly. Case closure letters will be sent to <br />Juvenile Services ale 'n them of the com letion of Home Based Services. Our <br />workers also attend JINenile Court when a juvenile we are serving has a scheduled appearance. Monthly <br />Progress reports are Ii,so sent to DJJ for documentation of our interventions. We maintain a positive working <br />relationship with Juvenile Services, our referral agent, and other resources such as mental health and the <br />schools. <br /> <br />FI5 <br />
