Involvement of the parent is l(n area of concern. Many youth are in trouble because of lack of parental involvement <br />in their life. Without parental s4pport, the program objectives may not be attained to the level desired. <br />2. Transportation is a problem for any child who does not drive or have a car. ALPHA is willing to set aside $400 for <br />public transportation for those who have no other alternative, however, each client is encouraged to work out <br />transDortation if Dossible and leSlve the funds for Dublic transDortation to those with the areatest need. <br />Documentation List the datl!i elements or records to be maintained: A) in confidential files to document <br />compliance and BJ to document, each measurable obiective. <br />1. All clients will have a confidential file established at the time of referral. All subsequent contacts are followed up <br />with individual notes reported b~ the therapist in Charge of the client's case. <br />2. The results of all assessmen(S, testing, profiles, etc. will be maintained in this confidential file. <br />3. A monthly report summary will be submitted to the appropriate court counselor and/or probation officer indicating <br />the progress of the client in the ALPHA program. <br />4. Access to the therapist is available on an as needed basis by the client, parent, court counselor and/or probation <br />officer. <br />5. Family attendance & particip"!tion will be documented in a confidential file. <br /> . <br />Treatment Element Descri~ what the component will do to redirect inappropriete youth behavior or how the <br />cO(f1ponent will impact inappropliate behavior. <br />Proper assessment of the clienl's behavior problem will help in identifying the root cause of the client's dysfunctional <br />or delinquent behavior.
